Note only ONE NRP form is issued for A/AA as per OMHA policy. Forms are automatically sent to intending centre and copy sent to requested email. Players need a new form every year.
AAA forms are for nearest AAA Centre Chatham Kent Cyclones: reminder if trying out for the first time please ensure you bring your players birth certificate and completed form.
